B0 - means that controller is SSS6688

Note=ISP:6688_B0-D3_20090929_V221_8433.BIN
Try this

Don`t enter the settings by password. (programm will crash)
Make changes by editing the 6688_Toshiba_43nm_D3_Normal.INI

it make this eror 8

Try to clean:
U3S SafeEraseUtility v1.16
After
SecureAP v1.0
or
MPTOOL 2.150

if not helps, try - test mode for controller before flashing
