Да как исправить то? Ведь проблема аппаратная, а не программная!
Я три недели убил на перепрошивку этой флешки, пробуя разные прошивки, бёрнеры google baseball и разные версии прошивальщика (Phison ST-TOOL)

Мое резюме: сюда воткнули изначально глюченный контроллер Phison в связке с проблемной памятью.
Как вариант: отпаять контроллер, заменить его на нормальный
